Date request forms are available via this link: https://forms.gle/12u86xPdBfszS42H7. No emails, faxes, or duplicates will be accepted. One guest per NCHS junior/senior is allowed. Date request forms are due by Friday April 15th, 2022 by 3:30 PM. NO EXCEPTIONS. The administration has the right to deny any guest from attending the NCHS prom at their discretion. Nobody 21+ may attend.
ALL WHO ARE ATTENDING MUST BRING A VALID SCHOOL OR STATE ID WHEN CHECKING-IN TO THE DANCE. No physical tickets are necessary - your name will be added to the list when you purchase online!
