Date request forms are available in the main office. No copies, faxes, or duplicates will be accepted. Copy of the guest's student ID or Drivers License MUST be submitted with every date request form. 1 Guest per NCHS junior/senior is allowed. Date request forms are due by Thursday, April 23rd, 2020 by 3:30 PM. NO EXCEPTIONS. The administration has the right to deny any guest from attending the NCHS prom at their discretion. Nobody 21+ may attend.
